Question 2002 civic 1.7 5mt problems

So, I just finished changing the timing belt on my car. I had a code for the crank position sensor, but that was the only issue. Until about an hour into my drive home, it threw both accessory belts. Any idea what could cause this, especially both belts at once? They were both tight, and my car is not out of time. Any ideas? Thanks!

The belt, pulley, and tensioner are all new and good quality. Any other ideas?

Use a straight edge and see where it's out of alignment. Something is not installed correctly.A washer missing or added in wrong place.
