Default Car problem, shaking from stop, Auto.

I already kind of have an idea of whats wrong, but I won't say anything so I won't give anyone ideas and all I get are posts of people agreeing with me. =D

Anyways, lately after my car is warmed up and has been running around for a bit the entire car shakes when starting from a dead stop. And when I am stopped it feels like the car is jerking back and forth.
Then when I let go of the brakes the car BARELY moves forward, I hit the gas and the revs go up, but I barely get any power. Theres a harsh shift then it kinda go back to normal

What should I do from here? This is my commuter car. I've been biking to work a lot more because of this too!

Thank you.
