Clutch went dry in Accord

Exclamation Clutch went dry in Accord

So my clutch has completely gone dry and the bandaid will no longer work. Before I could loosen the bleeder valve on the slave cyclinder for the clutch and pump fluid thru it until clutch started working then close valve and it would work for another week or so, not the story anymore. Need to know what the problem is, I DO NOT deal with clutches therefore I have no idea, but know my way around an engine so it shouldnt be hard helping me . Do I need to replace the hose going from cyclinder resevoir to slave cyclinder or could the slave cyclinder have a leak? replace everything to do with clutch? Car is a 95 with 169,XXX miles on it. please help!
