Default Engine mount Insulator (Driver side)

Putting everything back together and I come to this insulator. (50842-ST0-N10)
Notice the difference in shaft OD vs. the insulator ID.
Seems there should be another bushing or something in-between.

What am I missing here?

I don't remember it being like that when I took it apart - but that was a couple weeks ago.
I've been fairly diligent about separating and storing parts; but I only bagged two bolts and a nut for this. And my garage is a little busy but, again, I've been pretty good about keeping parts organized. I'd like to think I would see, remember, or find something as big as a bushing for this.

Hard to believe this insulator would do anything with that much gap. Inside the insulator, there's little or no wear. If it were supposed to be like this, it seems the shaft from the engine would beat the heck out of the inside of the insulator.

Does it go back together like this or is there something else that goes in with it?

perhaps the bolts for the left and right are mixed, diagram seems to appear the left bolt body is larger than the right.
