Default Clutz with a bad clutch

Ihope someone can help me. I have a '99 Foreman 400 and no matter ifI adjust the clutch loose or tight or if the machine is cold or warmed up it won't release whenI shift. It was fine when new. When I put it in gear it jumps and if I hold the shifter up when shifting into 2nd it is still engaged and I can move. If I hold it down when shifting back into first,it will move.If I loosen up the adj. a turn and a half from the recommended setting it sometimes breaks loose with a bang. I just don't want to blow it up. It has 650 milesand about 150 hrs. I used it in very low speed (walking speed) pushing wheeled carts (approx. 150 lbs)for most of its miles and I wonder if I burned up the clutch and it's grabbing. The idle speed is normal and I changed the oil and filter 4 times and it is always clean and not smelly.I'm an old time gear head and mechanic, is there a manual available to guide me if I have to tear this thing down? I think I can figure out the nuts and bolts of the matter, it's just any clearances or torque values I might need to have. Thanks in advance.
